Once you have fully completed the application, call the clinic
at 770-507-1344 to schedule a certification appointment.
IMPORTANT:
You MUST be an established Medical patient in order to receive expanded care, such as Dental and Mental Health services. Services such as dental, counseling, vision, specialties, etc. are not available as stand alone services.
